What?
This is a free event; invite a friend. Bring a water bottle and appropriate clothing & shoes depending on weather. There are some various food vendors at the Patch (like Red Leaf) so bring some money if you want to buy a pumpkin or get something while we are there.

I give my permission for the minor child named above to participate in all activities of the event named above and to be transported by a church vehicle (van or bus) when necessary.

I also authorize Calvary Community Church/events to use photos or videos taken of my child at events for promotion & advertising including but not limited to brochures, articles & websites.
